A recent national study reveals that nearly one in three young American women has never undergone cervical cancer screening. This is despite screening being a crucial preventative measure against a disease that causes approximately 4,000 deaths annually in the U.S. Published in JAMA Network Open, the study shows the percentage of women aged 21 to 29 who have never been screened rose from 12.5% in 2005 to 31.9% in 2023. The number of unscreened women increased from about 3.5 million to 6.2 million, with a significant rise after 2010.
Screening Guidelines for Cervical Cancer
Major medical organizations provide varied recommendations, all emphasizing HPV-based screening, as HPV infection leads to most cervical cancers.
U.S. Preventive Services Task Force (USPSTF)
- Under 21: No routine screening.
- Ages 21-29: Pap test every 3 years.
- Ages 30-65: Choose between Pap test every 3 years, primary HPV test every 5 years, or HPV/Pap co-testing every 5 years.
- Over 65: Screening may stop with adequate prior screenings and no high-risk history.
American Cancer Society (ACS)
- Begin screening at age 25 instead of 21.
- Ages 25-65: Favor primary HPV testing every 5 years.
- Alternatives: HPV/Pap co-testing every 5 years or Pap test alone every 3 years.
- Stop at 65 if the screening results are normal and no high risk is present.
Challenges for Older Women
The study also finds an increase in the unscreened rate among women aged 30 to 65, rising from 5.3% to 14.8% between 2005 and 2023. This increase from 4.3 million to 9.2 million women in this age group is concerning due to the higher cervical cancer risk during these years. Unscreened older women may discover the disease only at a later stage.
The findings use data from nine National Health Interview Surveys conducted from 2005 to 2023 and U.S. Cancer Statistics for 2001 to 2023.
Impact of Low HPV Vaccination Rates
Low HPV vaccination rates further exacerbate risks. Vaccination rates among women aged 21 to 29 grew from 5.7% in 2008 to 54% in 2019, yet 45.5% remain unvaccinated. About a third of unvaccinated women have never been screened, leaving them without crucial protective measures.
Trends in Cervical Cancer Diagnosis
Findings reveal an over 2% annual increase in cervical cancer diagnoses at regional and distant stages after 2017 among women aged 30 to 65. Although direct causation cannot be confirmed, the pattern underscores the importance of addressing screening gaps.
Disparities in Screening Access
The study identifies disparities in screening access. Women from racial minorities, those without insurance, and residents of the Southern states are less likely to have been screened. These groups already face higher risks of late-stage cervical cancer diagnosis and mortality, potentially worsening healthcare inequalities.
Despite limitations like self-reported data and lack of HPV status information, the research aims to spotlight trends needing more investigation. Addressing these healthcare gaps is critical for preventing cervical cancer, emphasizing regular screening and vaccination accessibility.
